The Surgeon is a serial killer who has been enacting the same crime for nearly 60 years. The killer has been evading capture and their identity is completely unknown. The murders are all the same: a man is kidnapped, and their face is skinned after enduring horrendous sexual torture. There has only ever been one survivor. Joshua Harding is that survivor. He is a struggling writer whose fame skyrocketed when he became a victim to one of the most prolific killers in history. His narcissistic tendencies are worsened after his face is taken from him by The Surgeon. His vanity drives him to extreme lengths in order to reclaim his stolen face. Samantha Allen has been working The Surgeon case for the past 20 years. Assisted by its only witness, Joshua Harding, she has finally started to gain an understanding as to who The Surgeon may be. Due to years of working the same case, she begins to have intense hallucinations, linked to her developing schizophrenia. Rotating between these three characters, The Surgeon case is blown wide open when the killer decides she wants to 'go public'. With Samantha's mental health deteriorating, whilst her obsession with the case intensifies, she races against herself to solve the crime before the thousands The Surgeon has killed turns into thousands more.
